In submitting this Direct Debit Request, I/we:

•request and authorise Lyft’d Pty Ltd  to arrange through its own financial institution a debit to my/our nominated card or account for the full amount of my/our monthly invoice for all services I have with Lyft’d Pty Ltd on the due date specified in my/our invoice, or otherwise in accordance with any alternative payment schedule or instalment plan you have agreed with us;

•understand that debit will be made through PayPal and is subject to our Service Agreement which is available upon request from Lyft'd.

•confirm I/we have authority to give Direct Debit instructions on the nominated card or account;

•authorise Lyft’d Pty Ltd to verify details of the nominated card or account with my/our financial institution, as necessary.
